Objective: This study investigates the efficacy and safety of epidural infiltration with drugs and an oxygen-ozone mixture for treating cervicobrachialgia due to disc-radicular conflict or on a degenerative basis, utilizing both retrospective analysis and direct visualization techniques. Methods: A retrospective study involving 10 patients treated with epidural infiltrations of an oxygen-ozone mixture and cortisone was conducted. The procedures were performed under CT guidance to ensure precise delivery and to monitor the diffusion of the injected substances. Pain levels were assessed using the Numerical Rating Scale (NRS) and treatment efficacy was evaluated based on symptom relief and reduction in NSAID intake. Results: Significant pain reduction was observed post-treatment, with median NRS scores decreasing from 9 (baseline) to 2 (follow-up), and a significant decrease in on-demand NSAID intake. Only one minor complication of a headache was reported. The study also demonstrated the ability of ozone to diffuse through the epidural adipose tissue, potentially enhancing treatment efficacy. Conclusion: The combined use of an oxygen-ozone mixture and cortisone for epidural infiltration is an effective and safe treatment for cervicobrachialgia, offering significant pain relief and minimizing the risk associated with traditional epidural injections. This technique presents a viable non-surgical option for patients suffering from disc-radicular conflict or degenerative conditions.
Objective: Sleep-wake disorders is a common disease in children and adolescents. In recent years, there has been an increasing number of studies on the intervention of exercise therapy in sleep-wake disorders. This study aims to systematically review the development status, research frontiers, research hotspots and development trends of exercise therapy in the through bibliometric methods. Methods: The data comes from the Web of Science Core Collection database. Select all the original data from the establishment of the database to 26 April 2024. Summarize the external characteristics of the literature through Web of Science, Use Excel 2021, Origin 2021, VOS viewers 1.6.20 and Cite Space 6.3.R1 to visually analyze countries/regions, institutions, journals, authors, co-cited references and co-occurrence keywords, use the bibliometric online analysis platform (https://bibliometric.com/) to analyze the changes of keywords and extended keywords over the years. Results: We received a total of 775 publications. The works were sourced from 1429 institutions in 75 countries/regions, published in 113 journals, and written by 4332 authors. The number of publications peaked in 2012, 2018, 2019 and 2021 respectively. In the United States, Harvard University and Children (Basel) have the highest number of publications in this field. The analysis of co-cited references shows that there are three main research frontiers in this field, including 24-hour exercise behavior guidelines for children and adolescents, COVID-19 lockdown and cardiometabolic risk. Screen time, mental health, validity, depression, guidelines, stress, and mediterranean diet are still the current research hotspots in the field, and may become potential research hotspots in the future. Conclusion: The development of research in the field of exercise therapy for children and adolescents with sleep-wake disorders is relatively slow, and there is still a lack of cross-regional scientific research collaborations between countries/regions, institutions and individuals. Our research suggests that it may be a worthwhile research direction to promote the establishment of healthy lifestyle behaviors in the gathering environment of children and adolescents, formulate targeted policies for disease prevention, diagnosis and management, strictly implement preventive measures, improve the level of diagnosis, and dig deep into the precise treatment plan of diseases.
The optimized methodology and results of the new characterization in terms of dose and image quality of the X-ray system used in the main pediatric hemodynamics service in Chile are presented. In addition, scattered dose rate values at the operator’s eye level are reported for all acquisition modes available in different thicknesses of absorbent media and angiography. The characterization was performed according to the European DIMOND and SENTINEL protocols adapted to pediatric procedures. The air kerma at the entrance surface (ESAK) was measured and the image quality parameters signal-to-noise ratio (SNR) and a figure of merit (FOM) were calculated. The scattered dose rate was measured in personal dose equivalent units. The ESAK for fluoroscopic modes ranged from 0.2 to 35.6 μGy/image when passing from 4 to 20 cm of polymethyl methacrylate (PMMA). For the cine mode, these values ranged from 2.8 to 160.1 μGy/image. The values of the image quality parameters showed a correct system configuration, although abnormal values were observed in the medium fluoroscopic mode. As for the scattered dose rate at the level of the cardiologist’s eyes, the highest value is PMMA with a thickness of 20 cm, where the cine mode reached 9.41 mSv·h-1. The differences found from previous evaluations can be explained by the deterioration of the system and the change of one of the X-ray tubes.
Agroforestry holds the key in providing alternative economically viable livelihood development and to support mountainous farmers to adapt to climate change. Innovative agroforestry interventions integrating animal production, horticulture etc into cropping systems exist that can help farmers improve yields and build resilience for supporting livelihoods particularly among marginal communities. But, the lack of knowledge, technical know-how and other information among the farmers are major barriers in adoption of agroforestry. Millions of the farmers of mountainous regions are already wrestling with water scarcity, which would be more severe in climate change scenario. The Himalayan regions are have been considered to be highly sensitive to climate change. Indeed, Innovative agroforestry interventions have the potential to conserve natural resources, improve productivity and provide resilience to climate change. The present paper highlights the need for developing innovative agroforestry interventions to promote various alternate livelihood options through diversification, adoption of high yielding varieties and development of innovative products from forest resources. Of these spice based agroforetry, silvi-medicinal systems, Van silk cultivation, bamboo and ringal cultivation and development and use of farm resources based products like bamboo based composite structures, Seabuckthorn herbal tea, Ghingaroo juice (Crataegus crenulata) and incense products etc holds a promising potential to be explored as better options for future scenario.
Diabetic retinopathy (DR) is a major cause of blindness globally. Effective screening programs are essential to mitigate this burden. This review outlines key principles and practices in implementing DR screening programs, emphasizing the roles of technology, patient education, and healthcare system integration. Our analysis highlights key principles for establishing successful screening initiatives, including the importance of regular screenings, optimal intervals, recommended technologies, and necessary infrastructure. We emphasize the roles of healthcare providers, patients, and policymakers in ensuring the effectiveness of these programs. Our recommendations aim to support the creation of robust policies that mitigate the impact of DR, ultimately improving public health outcomes and reducing the incidence of blindness due to diabetic retinopathy.
This study informs the academic and policy debate on the policy effectiveness of exchange rate interventions on exchange rate levels and volatility. Using a constructed data set comprising daily data on exchange rates, monetary policy fundamentals, exchange rate intervention dates and magnitudes of those interventions as well as financial news speculation of such interventions, we empirically estimate the policy effectiveness of Bank of Japan interventions in the exchange rate over the 12-year period between 2010 and 2022. This allows us to investigate the policy effectiveness of a variety of exchange rate interventions, or news of exchange rate interventions, across different time-horizons. We find that policy interventions in the yen exchange rate are more effective over short-horizons than long-horizons, more effective when the policy objective is a competitive devaluation of the yen rather than a revaluation, and more effective at influencing the level of the yen against major world currencies other than the US dollar. In fact, for the yen-dollar rate, we find that policy interventions may have the unintended consequences of weakening the yen (when the policy intention is to strengthen it) and increasing volatility in the yen-dollar exchange rate.
